Συχνότητα και παράγοντες κινδύνου κρίσεων "Ε" μετά θρόμβωση φλεβωδών κόλπων (Master thesis)

Purpose: In this retrospective study we analyzed the risk factors and the frequency of the seizures in the sinus Venus thrombosis (CVT).
Method: We analyzed four retrospectives studies and we compared the risk factors which depend on the part of the brain’s damage. The aim in that moment is to investigate whether, there were presenting seizures before the diagnosis of CVT and what kind of seizures there were.
Results: The frequency of presenting seizures was between 39,3% and 46,7%. The frequency of early seizures was 4,4% and 6,9%. Seizures appeared more often in children (71%) and in neonates (48%) compared with adults (37%).
The supratentorial lesions were associated with more than 4,67 times high risks of seizures. The frontal and parietal lobes lesions were associated with higher risk of presenting seizures. Seizures are more frequent in patients with focal deficits. Mortality rate observed in patients with seizure (12,5%) was higher than that observed in patients without seizures (6,3%).
The risk of earlier seizures decreased when prophylactic treatment was used (one patient with seizures in 148 patients treated by this method, compared with 25 patients with seizures in 47 patients without prophylactic treatment OR=0,006, 95% CI=0,001 + 0,005).
The important factors of earlier seizures in CVT are GCS <8 (P= 0,009) and the increase of D-DIMER (P=0,003)
Remote seizures were more often in supratentorial lesions and thrombophilia. The earlier seizures are the most important factor for the remote seizures (P value =0,000, OR:13.14, CI:2,54- 201.2).
Conclusion: Our results demonstrated that seizures in the acute phase are the most important factor for the development of remote seizures, associated with supratentorial parenchymal lesions. GCS<8 with elevated D-DIMER levels were also significant factors.
