Διερεύνηση των ευρημάτων του OCT (RNFL-GCC) σε ασθενείς που παρακολουθούνται στο τμήμα γλαυκώματος και έχουν φυσιολογικό οπτικό πεδίο, σε σχέση με την κλινική προσέγγιση και διαχείρισή τους (Master thesis)

PURPOSE: To investigate the role of OCT parameters (RNFL,GCC), in patients without detectable glaucomatous functional damage (VF) and the correlation with patients’ management. METHODS: Retrospective search of patients’ files, that belong to a glaucoma department, of a public hospital. Only patients with normal visual field tests were selected and the data were processed with statistical analysis. Three categories of patients’ management were formed: 1=medication for IOP lowering, 2=observation with follow-up tests, 3=discharge from glaucoma department. Case tests were performed, to investigate the possible role of the variables, in order to classify these patients in one of three teams. RESULTS: Sample variables with the strongest statistical significant relationship were RNFL1st with RNFLlast (r=0,91, p<0,0001) and GCC1st with GCClast (r=0,91, p<0,0001). The difference values between the last OCT testing and the first were presented with negative sign, so it seems that OCT parameters values are decreasing by time (RNFLdif:-3,13±3,26, GCCdif:-1,10 with IQR=3,07). Concerning the classification between the three categories of management, statistical significant relationship was found, only when separating team 1 from team 2 and also team 1 from team 3. This kind of relationship was found for variables like age, maximum IOP and for all OCT parameters (except for RNFL1st). There was no statistical significant conclusion when discriminating team 2 from team 3. CONCLUSION: Several known facts from literature were confirmed, about the subgroups of glaucoma that were tested (glaucoma suspects and ocular hypertension). OCT parameters were correlated with the case of medication taking. The chances that a patient would sort in team 1, were higher as OCT values were decreasing. Of course, OCT evaluation was not the only influence on management decision, as there are many established factors for such a hard decision making.
