Μελέτη της γωνίας προσθίου θαλάμου με Οπτική Τομογραφία Συνοχής Προσθίου Ημιμορίου (AS-OCT) σε ασθενείς που υποβλήθηκαν σε οπίσθια υαλοειδεκτομή με ένθεση αερίου C3F8 λόγω ρηγματογενούς αποκόλλησης αμφιβληστροειδούς (Master thesis)

Purpose: The purpose of this study is to evaluate by AS-OCT imaging the impact of posterior vitrectomy with gas tamponade on the anterior chamber (AC) angle, in patients with primary regmatogenous retinal detachment. Materials and methods: 57 patients who underwent posterior vitrectomy with C3F8 tamponade for regmatogenous retinal detachment were enrolled in this prospective study. Images of the anterior chamber were obtained by Visante AS-OCT pre-operatively and in 6 post-operative stages (1st, 5ih, 15ih, 30ih, 45ih and 90ih post¬operative days), before and after mydriasis (with tropicamide and phenylephrine). Parameters of the nasal and temporal AC angle (AOD500, AOD750, TISA500, TISA750, SSA), were graded using Visante's software. Additionally, IOP measurements were obtained in every examination, and axial length was measured pre-operatively and in the 90th post-operative day. All measurements were also obtained for the other - normal - eye. Results: In the study eye, statistically significant differences were found in all the parameters of the measured angle (AOD500, AOD750, TISA500, TISA750, SSA), between 1st and 5th postoperative day, as well as between the 5th and 30th, 45th, 90th postoperative days, on the nasal and temporal side of the AC angle in mydriasis and on the nasal side of the AC angle in miosis. Regarding intraocular pressure, statistically significant differences were found between the preoperative measurement and those of the 1st, 5th and 15th postoperative days, and between the 5th and 15th with the 90th postoperative day. Axial length is also a parameter that varies significantly, as it appeared to increase by 0.322mm on average. Finally, in the normal eye no significant change of parameters was observed, except for the intraocular pressure between the preoperative measurement and the 1st and 5th postoperative days. Conclusion: Posterior vitrectomy with gas (C3F8) tamponade appears to affect the morphology of the AC angle, by temporarily narrowing it. Intraocular pressure also increases, while the parameters return to baseline once the gas is absorbed. In addition, the angle changes do not differ in the groups of phakic and pseudophakic patients. Finally, regarding axial length, it seems that there is an - albeit small - increase.
