Η συμβολή του Enhanced Depth Imaging OCT και της OCT-Angiography στη μελέτη του χοριοειδούς σε ασθενείς με Χρόνια Κεντρική Ορώδη Χοριοαμφιβληστροειδοπάθεια πριν και μετά τη φωτοδυναμική θεραπεία (Master thesis)

The purpose of the study is to evaluate the choroidal changes after performing half¬dose photodynamic therapy (hd-PDT) in eyes with chronic central serous chorioretinopathy (CSC) using Enhanced Depth Imaging Optical Coherence Tomography (EDI-OCT) and Optical Coherence Tomography Angiography (OCT- A). Methods. This prospective case-control study included 10 eyes (10 patients). The choroidal layer was examined before applying hd-PDT (baseline) and at 1 month following treatment. Binarization with a modified method and analysis by ImageJ were applied at EDI-OCT and OCT-A images. We investigated the changes of the mean choroidal thickness (CT) at subfoveal area and at 750μm nasal and temporal of the fovea. The thickness of Haller and CC/Sattler layer were measured independently. Binarization method was applied to separately measure the whole area (WA), the luminal area (LA) and the interstitial area (IA) at EDI-OCT and the perfusion density (PD) at OCT-A. The ratio of the LA to the WA at EDI-OCT corresponded to the choroidal vascularity index (CVI). Visual acuity (VA) was measured. The unaffected fellow eye was used for comparison. Results. The mean CT concerning the subfoveal area and the area 750 μm nasal and temporal the fovea was significantly lower at 1 month after hd-PDT than the baseline (measurements of all p<0.001). The mean values of the Haller and CC/Sattler layer were significantly lower at 1 month after treatment than the baseline (measurements of all p<0.001). The values of WA, LA and IA were significantly reduced at 1 month post-treatment compared to the baseline values (measurements of all p<0.001). A statistically significant increase of the PD in OCT-A was observed at 1 month after hd-PDT compared to the baseline (p<0.001). CVI did not differ significantly before and after treatment (p>0.05) There was a significant negative correlation between the PD and VA before and after applying hd-PDT (measurements of all p <0.05). None of the analyzed parameters reached the values of the unaffected fellow eye at 1 month after hd-PDT. Conclusion. After applying hd-PDT, choroidal thickness at each measured area and thickness of Haller and CC/Sattler layer significantly declined, with both luminal and interstitial components to be markedly reduced. OCT-A is an objective tool for evaluating the values of choriocapillaris perfusion that seem to have an amelioration post-treatment.
