The effect of nasal operations as an isolated treatment of Obstructive Sleep Apnea (Master thesis)

Objective: Obstructive Sleep Apnea Syndrome (OSA) is one of the most common sleep disorders. Nasal obstruction plays a significant role in OSA symptoms and the severity of the disease. There is a controversy regarding the efficacy of nasal surgery as the only treatment for OSA. The purpose of this systematic review is to review the literature on the effectiveness of nasal surgery as an isolated treatment of OSA and if the improvement of the Apnea- Hypopnea Index (AHI) and Epworth Sleepiness Scale score is significant or not. Materials and Methods: A systematic review was performed by two independent researchers after searching the most popular electronic databases using specific criteria. Twenty – three studies were eligible and were reviewed. Results: AHI was improved in most of the studies, following a nasal operation as the only treatment of OSA, but in the majority of the studies, AHI was not significantly improved. As far as the Epworth Sleepiness Scale score (ESS), it was reduced significantly in the vast majority of the studies. Conclusion: Based on the data analysis, isolated nasal operations might be efficient for controlling symptoms of nasal obstruction, but there is no significant improvement of the AHI postoperatively in most of the studies. The conclusion is that the effectiveness of nasal surgery as an isolated treatment for OSA is not well supported. On the other hand, subjective symptoms and measurements (ESS score) were improved. Consequently, sleep and life quality were also improved. Furthermore, nasal operations benefit patients with OSA who use CPAP because the nasal airflow is improved, and compliance with the CPAP machine is better postoperatively. We recommend an algorithm for the scientifically supported indications for referral and nasal operations in patients with OSA. We also believe in the importance of a Multidisciplinary team in every Hospital with a sleep disorders clinic for the holistic management of patients with OSA, and sleep disorders in general.
