Adherence and compliance to positive airways pressure (cpap) therapy in patients with sleep apnea and comorbid psychiatric disorders (Master thesis)

Obstructive sleep apnea (OSA) is a highly prevalent disorder in patients with serious psychiatric diseases (major depressive disorder (MDD), bipolar disorder (BD), and schizophrenia and psychotic disorder). However, this population is rarely diagnosed and treated and there are no large-scale studies showing evidence of adherence and successful treatment with PAP. We evaluated the effectiveness and the adherence to CPAP therapy in OSA, in individuals with comorbid psychiatric disorders (n=220, Group A) compared to individuals with non-psychiatric diseases (n=651, Control Group B). All participants were diagnosed with moderate/severe OSA, with overnight polysomnography and referred for PAP therapy. The two groups did not differ in terms of age and neck, waist, and hip circumferences. Psychiatric patients had a greater BMI (Body Mass Index (BMI) 32.8±6.5kgr/m2 versus 31.8±6.3kgr/m2) and they used to smoke more often (23.2% vs. 12.1%). After 3 months, similar percentage of both groups attended the clinic appointment for re-evaluation (n=41-20.4% psychiatric vs. n=220-25.3% non-psychiatric, p=0.086). CPAP usage was identical per night (p=0.267) and equally effective in normalizing Apnea Hypopnea Index (AHI) (p<0.001, Figure 1). In terms of alleviating daytime sleepiness, as expressed by Epworth Sleepiness Scale (ESS), CPAP was effective only in non-psychiatric patients (p<0.001), but not in psychiatric (p=0.833), perhaps suggesting the role of sedative medications in this population. In conclusion, OSA is treatable and PAP feasible in individuals with chronic psychiatric disease, however is not effective in decreasing daytime sleepiness .
