Comparison of a home sleep study based on pharyngeal and esophageal manometry with other types of sleep studies in patients with sleep apnea (Master thesis)

Introduction The use of a sleep study is of paramount importance for the evaluation of patients with Sleep Disordered Breathing. The evolution of technology in medical devices has succeeded in incorporating multisensory catheter manometry to a home-based sleep study. Aim The aim of the study is to present the use of the ΑpneaGraph® sleep study (Spiromedical, Norway) with multisensory catheter manometry as a diagnostic tool in patients with obstructive sleep apnea, to compare it with other types of sleep studies and to propose its role in the workflow of a sleep physician. Materials and Methods. Patients with symptomatology of obstructive sleep apnea were tested with ApneaGraph® sleep study The results calculated by the ApneaGraph® device were compared with validated devices i.e. the ApneaLink Air® (ResMed,USA) and the Embletta Gold® X100™ (Embla Systems, Inc). Results. A total of 50 patients were tested with ApneaGraph® and 21 of them were retested with either of the two validated devices. The results from the ApneaGraph® study correlate in a statistically significant way with the results from the other two sleep studies. The patients tolerate the sleep study reasonably well.
Discussion-Conclusion. A home-based sleep study device that incorporates multisensory catheter manometry can measure the obstructive episodes during sleep comparably to Polysomnography (gold standard examination) and, based on the literature, it can identify the level of obstruction similarly to the Drug Induced Sleep Endoscopy. Despite its limitations, it has advantages over other home-based sleep studies, as it can be used both as a diagnostic/screening study and as a preoperative tool in patients with Obstructive Sleep apnea.
